NOTICE TO BIDDERS FOR FY17 TELEVISION INSPECTION OF SANITARY AND STORM SEWERS TIME AND PLACE OF BID OPENING Sealed proposals with the name of project, date and time of bid opening on envelope for the improvement described here will be received at the Office of Village Clerk, 201 South Bloomingdale Road, Bloomingdale, Illinois, until 11:00 a.m. on Tuesday, October 18, 2016. Bid proposals submitted shall be opened and read aloud shortly thereafter. DESCRIPTION OF WORK The proposed improvement is officially known as "FY17 Television Inspection of Sanitary and Storm Sewers" which generally consists of the cleaning and closed circuit television inspection of approximately 29,770 feet of various size sanitary main, along with 19,600 feet of large diameter sanitary sewer collector pipe; and 17,645 feet of various size storm sewers located in the Village of Bloomingdale, Illinois to determine the existing sewer pipe conditions. SITE INSPECTIONS Pre-bid site inspections are recommended and available Monday through Friday from 8:00 a.m. to 3:00 p.m. on an appointment only basis. INSTRUCTIONS TO BIDDERS Specifications and proposal forms may be obtained from the Village Clerk at 201 South Bloomingdale Road, Bloomingdale, Illinois, 60108. The successful Contractor shall comply with the Illinois Prevailing Wage Act requirements (820 ILCS 130/1-12) for this contract including submission of certified payroll requirements and the employment of Illinois workers during periods of excessive unemployment. All Proposals must be accompanied by a Bank Cashier's Check, Bank Draft, Certified Check or Bidder's Bond, made payable to the Treasurer of the Village of Bloomingdale, for not less than five (5) percent of the total bid. All bidders are required to furnish a Flash drive and/or DVD inspection report along with a minimum of 3 municipality references with their bid. Inspection reports, Flash drive, DVD's and municipality references can be waived to bidders that have previously performed under this contractual service in the past. This Flash drive and/or DVD is to show the audio and video quality of their televising work to be performed. All bidders are required to submit a copy of their confined space entry policies, procedures and entry permits with their bids. PERFORMANCE AND COMPLETION BONDS The successful Contractor shall be required to furnish a Performance Bond acceptable to the Village in an amount equal to one hundred percent (100%) of the price bid for services for the term of the Contract (sample bond provided on page numbers 19 and 20). The Performance bond shall guarantee the performance of the Contractor and payment of all Contractors obligations under the terms and conditions of the specifications. The Performance bond shall be provided by a surety company acceptable to the Village and rated at least 5A by Best's Rating Guide. The bid security shall be forfeited if the bidder fails, or refuses to enter into a contract, within 10 days of the award. CERTIFICATE OF INSURANCE The successful contractor shall also furnish a Certificate of Insurance; the said policy and certificate shall name and show the Village of Bloomingdale as an additional insured.
