INVITATION FOR BIDDER'S PROPOSALS VILLAGE OF LIBERTYVILLE 2013 SANITARY-STORM SEWER TELEVISING PROJECT PROJECT NUMBER 1309 OWNER: Village of Libertyville 118 West Cook Avenue Libertyville, Illinois 60048-2090 1. Invitation to Bid: Owner invites sealed Bidder's Proposals for Sanitary-Storm Sewer Televising at various Village locations. 2. The Bid Package: The Bid Package consists of the following documents, all of which are by this reference made a part of this Invitation for Bidder's Proposals as though fully set forth herein: (1) Invitation for Bidder's Proposals; (2) General Instructions to Bidders; (3) Special Instructions to Bidders; (4) Addenda, if issued; (5) Bidder's Proposal; (6) Bidder's Sworn Acknowledgment; (7) Bidder's Sworn Work History Statement; (8) Form of Bid Bond; (9) Other Information Submitted by Bidder, if requested; (10) Notice of Award; and (11) Contract, including all of its Attachments and Appendices, if any. 3. Inspection and Examination: The Bid Package, including a more complete Invitation for Bidder's Proposals, may be purchased at the office of Owner's Village Engineer, 200 East Cook Avenue, Libertyville, Illinois 60048-2090, upon payment of $20.00 per set non-refundable fee. In making copies of the Bid Package available to prospective Bidders, Owner does so only for the purpose of obtaining Bidder's Proposals and such provision does not confer a license or grant for any other use. Each prospective Bidder shall, before submitting its Bidder's Proposal, carefully examine the Bid Package. Each prospective Bidder shall be required to make a mandatory inspection of the Work Sites and the surrounding areas and shall familiarize itself with all local conditions, including subsurface, underground and other concealed conditions, affecting the Contract, the Work and the Work Sites. The Bidder whose Bidder's Proposal is accepted will be responsible for all errors in its Bidder's Proposal including those resulting from its failure or neglect to make a thorough examination and investigation of the Bid Package and the conditions of the Work Sites and the surrounding areas. 4. Bid Opening: Owner will receive sealed Bidder's Proposals for the Work until 10:00 o'clock, a.m., local time, Tuesday, May 28, 2013, at Owner's office located at 200 E. Cook Avenue, at which time, or as soon thereafter as possible, all Bidder's Proposals will be publicly opened and read aloud. Bidders or their agents are invited to be present. 5. Bid Security, Bonds and Insurance: Each Bidder's Proposal shall be accompanied by a security deposit of at least five percent (5%) of the Bidder's Price Proposal in the forms more specifically identified in the Bid Package. Upon award of the Contract, the successful Bidder will be required to furnish a Performance Bond and a Labor and Material Payment Bond as more specifically identified in the Bid Package, and certificates and policies of insurance as required by Section 4 of the Contract.
